1095 Statement Available Upon Request (Calendar Year 2024)
A 1095 is a tax form employers use to report health coverage offered by an employer. It is part of the Affordable Care Act requirements.
As a result of the Paperwork Burden Reduction Act, individuals are no longer required to provide a 1095 as proof of health insurance coverage when filing your taxes. While we will not be mailing out forms, you may receive a copy of your 1095 form upon request.
